THE EFFECTS OF A MODERATE-INTENSITY CIRCUIT-TRAINING EXERCISE PROTOCOL ON WOMEN WITH HYPOTHYROIDISM: A CASE REPORT Kayla Knight, SPT Dr. Rob Sillevis, PT, DPT, Ph. D, OCS, FAAOMPT, MTC; Dr. Jacqueline van Duijn, PT, DPT, OCS Florida Gulf Coast University, Department of Rehabilitation Sciences, Fort Myers, FL, USA Introduction Clinical Impression Outcomes • Hypothyroidism (HT) is the state of an underactive thyroid and is the second most common type of endocrine disorder predominantly affecting women of reproductive age with a 5 -20 times higher incidence rate than men. 1, 2 • She presented with several symptoms related to hypothyroidism and had poor body composition in the form of obesity and poor cardiovascular health related to her low VO 2 max and hypertension. • Common manifestations of HT are reduced metabolism, weight gain, fatigue, decreased cardiac output, decreased exercise capacity, increased cardiovascular risk (e. g. Hypertension and/or vascular issues). 3, 4 • It was expected that there would be no adverse effects on her HT and hypothesized that the proposed exercise protocol would have a positive effect in improved baseline measurements of health and symptoms, thus supporting the use of therapeutic exercise as a potential method of treatment for HT. • There are currently no validated evidence-based parameters of exercise for the female population with hypothyroidism. 5 Current research of exercise tolerance with HT in general is scarce. • Based on reviewed literature, it appears that circuit training at a moderate intensity, combining strength exercises with aerobics is an appropriate exercise regimen to trial with HT patients. 6, 7 Patient History/Systems Review • The case subject was a 56 -year-old female with a long-time history of health struggles since her initial diagnosis with Hypothyroidism sixteen years ago. • This included thyroid hormonal imbalance, high blood pressure that began seven years ago, and difficulties with increased weight gain/losing weight. • The subject also has experienced other symptoms such as depression, fatigue, dizziness and concentration issues. • Upon baseline testing, it was established that she was in a deconditioned state. Intervention Week 1: Baseline testing Weeks 2 -9: 8 -week exercise intervention moderate-intensity circuit-training • Pre-participation vitals (BP, HR) • Individualized resistance measured at moderate intensity (12 -14 using Borg RPE scale) Week 10: Post Intervention testing • She has been unable to adhere long-term to any regular exercise program • She was on the hormone replacement medication NP Thyroid 120 milligrams at the time of the examination, and consented to participation. Examination • Thyroid Symptom Rating Questionnaire symptoms, waist-tohip ratio, weight, pre/post VO 2 Max using the six-minute walk test (6 MWT), blood pressure (BP), and heart rate (HR) data was collected. SRQ)8 Stage 2 Mixed Circuit • The subject completed a 15 -symptom survey using the Hypothyroidism Symptom Rating Questionnaire (Thy-SRQ questionnaire). 8 • The highest scores of symptoms bothersome to the participant on the Thy. SRQ 8 were given to tiredness, weight gain, memory problems, concentration issues, dizziness, and depression. Stage 3 • The subject reported experiencing 9 of the 15 symptoms included in the survey in some capacity. Variable Measurement Weight 211 lbs BMI 32. 2 WHR . 86 BP 1 133/80 mm. SRQ 8 symptoms such as dizziness, depression, concentration and weight gain were reduced the greatest (3 to 0).
