Documents Required by the City • City SWPPP Permit Application • Notice of Intent (NDDH Document) • SWPPP Template (NDDH Document) • Erosion & Sediment Control Pages from Plans *The Notice of Intent and the SWPPP Template are not required by the City for site disturbance less than 5, 000 sf.

City SWPPP Permit Applications will be rejected without: • Permit Application Fee • Owner

Notice of Intent • Required by the NDDH for construction sites 1 acre or

Notice of Intent • Required by the NDDH for construction sites 1 acre or larger. • Use the most recent version posted to the NDDH website. • Must have all necessary signatures. • The original must be sent to the NDDH, and a copy is submitted to the City of Grand Forks.

SWPPP Template • A copy must be on site during construction. • A copy

SWPPP Template • A copy must be on site during construction. • A copy does not need to be submitted to the NDDH unless requested by the department. • Use the most recent version posted to the NDDH website.

Erosion and Sediment Control Plans • Use to show the location of all BMPs

Erosion and Sediment Control Plans • Use to show the location of all BMPs (inlet bags, silt fence, construction entrance, etc. ) • Show before and after contour lines on sites greater than or equal to 1 acre. • Include a copy with the SWPPP located on site to be updated when necessary. • Provide the City of Grand Forks with a copy of any changes that are made to the plans.

Notice of Termination City Projects • Submit to the City when the City Inspector

Notice of Termination City Projects • Submit to the City when the City Inspector has determined the site to be stabilized. • If approved by the City Engineer, the N. O. T. will be sent to the NDDH. Private Projects • Send to the NDDH when the site has been stabilized and approved by the City Inspector. • Provide a copy to the City with the Owner’s signature
