Attention All School Bus Drivers • All commercial drivers are required to provide the DPS with a “Self. Certification Affidavit” (CDL-7)

Forms to Use • CDL-7 Category 2 of the four categories should be the choice for a driver who drives a school bus, but does not drive over-the-road tour buses, semi tractors or across state lines

• CDL-10 This form is an exemption form which allows a school bus driver to maintain a CDL without showing proof of a DOT Physical card every year This form must be included with the CDL-7

A Notary is Necessary • The CDL-10 will need to be notarized before submitting the document • It will be rejected if is not completed correctly

Loss of CDL • Once downgraded, you will be required to complete all knowledge and driving tests to reinstate your CDL
