Exciting Changes for Nurse Aide State Testing • Moving away from five individual performance of tasks • Moving towards fluid scenarios to mirror clinical setting • All skill task scenarios Include evidence-based practice on hand hygiene.
Skill Changes • Changing the skill steps that state “identify hands should be washed” • Replace with: “Perform hand hygiene. a. Cover all surfaces of hands with hand sanitizer. b. Rub hands together until hands are completely dry. ” • Removed the hand washing as a separate skill task and embedded it with one of four mandatory first tasks.
Mandatory First Scenario with Embedded Handwashing • Every student will get one of the following scenarios which include embedded handwashing at the end of the scenario: – Bedpan and output with hand washing – Catheter care with hand washing – Isolation gown/gloves emptying urinary drainage bag with hand washing – Perineal care of a female with hand washing
At the end of the first scenario, the students will perform the following hand washing steps: Washes hands: Turned on water. Washes hands: Wets hands. Washes hands: Applies soap to hands. Washes hands: Rubs hands together using friction. Washes hands: Rubs hands together for at least 20 seconds. Washes hands: Interlaced fingers pointing downward. Washes hands: Washed all surfaces of hands and wrists with soap. Washes hands: Rinses hands thoroughly under running water with fingers pointed downward. Washes hands: Dries hands on clean paper towel(s). Washes hands: Turns off faucet with a clean dry paper towel (s) (Does not use a previously used paper towel to turn off the faucet). Washes hands: Discards paper towels into trash container as used. Washes hands: Does not re-contaminate hands at any point during the procedure.
Additional skill scenario tasks: • • • Ambulation with a gait belt Ambulation with walker Backrub Blood pressure Denture care Dressing a resident Feeding a dependent resident Hair care Making an occupied bed Making an unoccupied bed Mouth care – brushing teeth Mouth care of a comatose resident • Nail care one hand • Partial bed bath – face, arm, hand underarm • Position resident on side • Range of motion for hip & knee • Range of motion for shoulder • Stand/pivot/transfer with a gait belt • Stand/pivot/transfer wheelchair to bed using a gait belt • Vital signs – oral temperature, pulse and respirations • Vital signs – axillary temperature, pulse and respirations • Weighing an ambulatory resident
Training Programs and Test Sites will need: • There will be one additional item that training programs and test sites will need to prepare students for these new skills which is: – Hand sanitizer • CDC recommends using ABHR with greater than 60% ethanol or 70% isopropanol in healthcare settings.
