Mandatory Submission Format / Tender Check List – File 3, 4 & 5 • Proposal Submission File no. 5 – Response to Section A: Adding Service Providers on the existing panel of Service Providers (Phase 1) should read Response to Section B: Appointing new Panel of Service Providers.

TENDER SCOPE OF WORK Section A – Adding Service Providers on the existing panel of Service Providers (Phase 1) Clarification: • • Only one CV is required per role, the Resource CV Response Template should be used for all CV’s. Administrative Requirements – • Functional and Technical Requirements – • Bidders must have a minimum of five (5) years proven experience in providing the required services; Functional/Technical evaluation threshold is 75%. Evaluation Criteria – Section 3 (Bidder’s human resources Professional Certification) and Section 4 (Bidder’s human resources Formal Qualification) indicates that “The score is allocated per CV (2 per role) “, this does not mean that two CV’s are required, the score for each CV will be multiplied by 2 to cater for the minimum and maximum points.

TENDER SCOPE OF WORK Section B – Appointing new Panel of Service Providers. Clarification: • • Only one CV is required per role, the Resource CV Response Template should be used for all CV’s. Resources consent should be obtained for every CV submitted, the “Cor 34 -2016 Annexure C IT Panel Phase 2 - Resources Consent Letter” refers to COR 37/2016, this should be COR 34/2016, the updated document will be distributed to all bidders. • Administrative Requirements – • Functional and Technical Requirements – • Bidders must have a minimum of five (5) years proven experience in providing the required services, these services should have been conducted without interruption for the last (5) years; Functional/Technical evaluation threshold is 70%. Evaluation Criteria – – Section 3 and 4 - In cases where the formal qualifications and professional/technical certifications were not requested, every bidder will be allocated minimum points. Section 5 - The turnaround times will be included on all successful bidder’s resourcing agreement based on their tender response.

TENDER SCOPE OF WORK Section B – Appointing new Panel of Service Providers. Clarification: • Annexure A 2: Planning and Control Category has a role “ 4. IT Asset Controller” however under the Cor 34 -2016 Annexure E 2 IT Panel Phase 2 - Planning & Control Role Profiles document (page 7) the job profile listed is for “IT Commercialisation Specialist”. The job profile for IT Asset Controller was not included. The updated Cor 34 -2016 Annexure E 2 IT Panel Phase 2 - Planning & Control Role Profiles_v 2 will be distributed to all bidders.
