Teacher Evaluation Formal and Informal Observation Cycle Process Stage 2: Planning Conference Stage 1: Prior to Planning Conference • O reviews D 1 data, and Teacher • Schedule planning conference which notifies T to complete Pre-Observation Form • Documents data for D 1 components (Form: Pre. Observation Form ) • Attaches optional lesson plan and artifacts of specific planning for observation • Submits back to O no later than one (1) work day before planning conference Observer optional lesson plan, and artifacts • Determines questions or additional information needed for D 1 data • O and T collaboratively review and extend D 1 data during planning conference as needed; O can document additional data collected at planning conference Teacher & Observer Stage 4: Reflection Stage 3: Observation and Data Conference Review Teacher • During observation, collects data for D 2 & D 3 • Sorts data and sends to T to review • Submits data to T at least two (2) days before the reflection conference to review* which notifies T to complete Post. Observation Form • Schedules reflection conference Observer • Documents evidence for D 2, 3 and 4 a based on review of O’s data and self-reflection of lesson* (Form: Domain 2, 3, and 4 a Post-Observation Form ) • Submits back to O no later than one (1) work day before Reflection Conference • Attaches or brings to conference artifacts of student learning (Limit 3) • O reviews Post-Observation Form before reflection conference • O & T review and confirm data and artifacts of student learning; O can document additional data collected at reflection conference • AFTER REFLECTION CONFERENCE • O finalizes D 1, D 2, D 3 and D 4 a data in observation data • O aligns critical attributes to D 1, D 2, D 3, and D 4 a components • O confirms/submits completed data set for observation cycle Teacher & Observer
