The 2567 ( CMS Statement of Deficiencies) will be sent to the facility within 10 working days of the last date the surveyors are on cite. Plans of Correction When the Regional Manager upload the 2567 into KOTA an email will be generated to the Administrator on Record to notify the facility that the Plan of Correction can be electronically loaded into KOTA. The Plan of Correction must be electronically uploaded into KOTA by the 10 th CALENDAR day of the RECIEPT of the 2567. Follow the Plan of Correction Brochure that is provided to the facility in the email that is sent with the 2567. November 22, 2020

Desk Revisit Process When the RM determines a facility is eligible for a desk revisit an email will be sent from KDADS stating this. Once the facility has been notified they are eligible for a desk revisit the following items are the responsibility of the facility: November 22, 2020

Facility Responsibilities for Desk Revisit Submit all required documentation to support the alleged compliance as outlined in the Plan of Correction for EACH F tag. November 22, 2020 The documentation needs to be submitted electronically in KOTA no later than 45 days.
