Substance Use Intervention Team Workflow In-Patient Nurses • 2 -Question screening given to in-patients on Medical Floors in the Hospital • Question 1: • Men: How many times in the past year have you had 5 or more drinks in a day? • Women: How many times in the past year have you had 4 or more drinks in the same day? • Question 2: • How many times in the past year have you used a recreational drug or used a prescription medication for nonmedical reasons? In-Patient Social Workers • Full AUDIT/DAST Questionnaire completed • Alcohol: Patients who answer “ 1 or more” should receive a full alcohol screen (AUDIT) • Drugs: Patients who answer “ 1 or more” should receive a full drug screen (such as the DAST). * Addiction Medicine Consult • The In-patient consult team will round on identified patients • Medication Assisted therapy • SUIT SW/RN Coordinator will be responsible for orchestrating appointment to outpatient Clinic and other Resources Outpatient Clinics • Patients can be referred to outpatient clinic for follow up, psychotherapy and Medicine assisted therapy • Outpatient Clinics: • Rush Addiction Medicine Clinic (Patient must have a Dual Psychiatric Diagnosis) • Toxicology Clinic (This will be a medicine clinic available to patients who do not have an existing psychiatric diagnosis. ) 2

Patient List Column “Substance Use” Case Manager to add this new column manually Legend of Substance Use column (from top to bottom) • • • RN Substance Use Screen is not done or documented RN Substance Use Screen – Negative (answered with “None”), meaning done with the patient. RN Substance Use Screen – Positive (answered with “ 1 or more” for any of the RN questions), meaning Case Manager needs to see the patient. Case Manager AUDIT and DAST screen is done and documented. Patient’s AUDIT score is 20+ or DAST score is 6+ that indicates IV-severe zone which requires Consult to Addiction Medicine, but the consult order is not placed. Once the consult is ordered, the column turns green check. 4
