Delivery Process of Impact Aid Forms • Impact aid forms are delivered to each school 1 to 2 days prior survey date • Teachers will forward tabulation of impact aid forms per class to principal’s office by October 4 th • Letters and guidelines are delivered to principals and teachers • Principal will forward a tabulation of impact aid forms for the entire school to Student Welfare by October 12 th • Teachers will send impact aid forms home with students on September 20 th • Impact aid forms are returned to teachers by September 28 th • Student Welfare will review impact aid forms and compile report • Impact aid report will be forwarded to the U. S. Department of Education on January 31 st 7
