In the event that you are the first to identify a emergency situation: • Dial *55555 (WCH) • Give your name, exact location, and type of emergency. • All emergency notifications will be announced 3 times over the audible paging system by the switchboard operator indicating the Code (type of event) and the Zone (area of the hospital experiencing the code situation).

Code Red - FIRE • In the event of a fire occurring in the

Code Red - FIRE • In the event of a fire occurring in the hospital, a “Code Red” is announced. • If you encounter a fire, dial *55555. *Remember to state the type of code, and provide your name and exact location. • If you hear a “Code Red” announced overhead, remain calm and wait for directions. • Contact your instructor to report your location and receive further direction. • Students are not expected to respond.

Code Blue – Cardiac Arrest • In the event of a cardiac arrest occurring in the hospital, a “Code Blue” is announced. • In the event that you encounter a person who is not breathing or does not have a pulse, dial *55555 or 1111. *Remember to state the type of code, and provide your name and exact location. Wait for the operator to repeat this information back to you. • If you hear a “Code Blue” announced overhead, remain calm and wait for directions. • Contact your instructor to report your location and receive further direction. • Students are not expected to respond.

Code Stork – Imminent Delivery WCH ONLY • In the event of an imminent, emergency delivery occurring in any area other than the Labor and Delivery Unit, a “Code Stork” is announced. • If you encounter a mother who is in need of assistance due to imminent, emergency delivery, call *55555 or alert someone to do so. *Remember to state the type of code and provide your name and exact location. • Stay calm and remain with the mother until help arrives.

Code Pink – Infant or Pediatric Abduction • In the event of a suspected or actual infant or pediatric abduction occurring in the hospital, a “Code Pink” is announced. • Students are to review the documents provided on Infant and Pediatric Security. • If you hear a “Code Pink” announced overhead, remain calm and wait for directions. • Contact your instructor to report your location and receive further direction.

Code Black – Bomb Threat • In the event of a bomb threat in the hospital, a “Code Black” is announced. • If you receive a bomb threat call, listen carefully to the caller, record as much information as possible, and signal someone to call *55555. • If you hear a “Code Black” announced overhead, remain calm and wait for directions. • Contact your instructor to report your location and receive further direction. • Students are not expected to respond.

Code Grey – Inclement Weather • A Code Grey Alert will be called if the Lafayette area is placed under an Inclement Weather/Tornado Warning. • Code Grey Activated will be announced if inclement weather/tornado poses imminent danger to the hospital. *Students should immediately go to the hallway and protect themselves from harm if a “Code Grey Activated” is called. • Code Grey All Clear will be announced when the weather event is no longer a risk to the hospital.

Code Yellow- External Disaster • In the event of a disaster (within the community or the hospital) resulting in injuries or illnesses which exceed the hospital’s normal capability to render routine care, a “Code Yellow” is announced. • If you hear a “Code Yellow” announced overhead, remain calm and wait for directions. • Contact your instructor to report your location and receive further direction. • Students are not expected to respond.

Code Green - Evacuation • In the event that an evacuation of all or part of the hospital is needed, a “Code Green” will be announced. • If you hear a “Code Green” announced overhead, remain calm and wait for directions. • Contact your instructor to report your location and receive further direction.

Code Silver – Active Shooter • In the event of an active shooter in the hospital, a “Code Silver” is announced. • In the event of a Code Silver, students should do one of the following: – Leave the building and contact your instructor immediately to inform them of your location. – Enter a S. A. F. E. room (locked S. A. F. E. rooms are located on all nursing units). – Hide in any available area in the facility. – Defend yourself. Physically defending yourself should only be done as a last resort. *Do exactly as the team of law enforcement officers directs, and contact your instructor to inform them of your location as soon as it’s safe to do so.

Restraints • Restraints are only used when appropriate to provide safety to patients. • Any restraint use poses a significant threat to the patient. • You should never release a patient from restraints without notifying a nurse.

Scissor Safety Do Not use scissors to remove dressing material, tape or securement devices

Scissor Safety Do Not use scissors to remove dressing material, tape or securement devices at or near an infusion site. • Do Not use scissors when another, non-sharp approach can be used. • MINIMIZE and AVOID the use of scissors on the neonatal, pediatric or elderly patients. ¢ Cut only what can be seen. If the bandage is thick, cut one layer at a time, after removal of tape. ¢ • Use clear/transparent dressings so that the area can be fully visualized. ¢ • Use a minimum amount of tape necessary to secure a dressing or arm board. ¢ • Have a second person secure the body part and digits prior to cutting when the patient is unable to keep still or if the situation warrants • Use a shaver/clipper to remove body hair rather than scissors. • Consider use of Montgomery straps when bulky, frequently changed dressings are required. ¢ • Place scissor blades so as to cut away from the body/extremity surface with the blunt side of lower scissor blade touching the body surface, positioning the cutting surfaces of the blades at 90 degrees to the body surface, rather than parallel. • Confirm the location of body part and all digits/appendages in the area prior to cutting. ¢ • Use blunt-edge scissors • Apply acetone-free adhesive tape remover (i. e, detachol) to address difficulties in removing tape, rather than cutting the tape. • Cut from the larger, proximal portion of an extremity toward the direction of the distal, narrower portion.
