2016 – 17 Wellness Program Requirements • Complete activities 1 and 2 by March 15, 2016 to avoid a $600 surcharge • Activity 1: Online Health Assessment • Activity 2: Biometric Screening • Complete activity 3* by May 16, 2016 to earn up to a $600 wellness credit • Activity 3: Complete one or more personal health goals * Activities 1 and 2 must be completed to be eligible for Activity 3

1. Complete your Online Health Assessment Complete the online health assessment between January 1 and March 15, 2016

Online Health Assessment The health assessment takes most people about 10 minutes to complete.

2. Complete Your Biometric Screening Option 1: Onsite screening Option 2: Preventive exam with your doctor Option 3: Ministry Medical Group Occupational Medicine Complete your biometric screening between January 1 – March 15, 2016

3. Complete Your Personal Health Goals • Option 1: Set a goal using My Health Assistant • Option 2: Online Health Challenge • Spring 2016 – more details to come • Complete Personal Health Goals by May 16, 2016

2015 – 16 Wellness Program Reminder • There’s still time to earn your $600 wellness credit from last year’s wellness program • To be eligible you must have completed the online health assessment and biometric screening between January 1 and March 15, 2015. • Complete two personal health goals by May 16, 2016. • Set one or more goals using My Health Assistant. • Complete the spring online health challenge.
