Complete current Safety Form Have parent sign Safety Form Submit parent-signed Safety Form with Research Plan via email STEAMFest. Forms@kirkwoodschools. org Include details in your logbook on exactly how you followed the safety guidelines The ten-page limit on your logbook includes both Safety Form and Research Plan REMEMBER: the Safety Form is worth 10 points on your project

Allergies: If enlisting people to help or be test subjects, always ask about allergies and use the Human Informed Consent form Animals: No animals should be harmed or caused pain Bacteria: No projects that involve collecting and growing bacteria cultures Guidelines Eye Protection: Safety glasses should be used for experiments with chemicals, or if any kind of splash may come in contact with your eyes Fire: Grades K thru 5 are not allowed to include fire or burning objects in their display Gloves: Safety gloves should be used for any testing with food or chemicals Humans: No experiments that can cause potential harm to humans Recording: All safety procedures need to be recorded in your logbook
