LAW ARS 13 -3620 • 13 -3620. Duty to report abuse, physical injury, neglect

LAW ARS 13 -3620 • 13 -3620. Duty to report abuse, physical injury, neglect and denial or deprivation of medical or surgical care or nourishment of minors; medical records. • Any person who reasonably believes that a minor is or has been the victim of physical injury, abuse, child abuse, a reportable offense of neglect that appears to be inflicted upon the minor by other then accidental means.

A report is made to you • *Someone reports to you that a minor

A report is made to you • *Someone reports to you that a minor has been injured or abused by someone else. • *The person reporting to you maybe the student or a friend of the student or a community member. • *YOU are to use the form listed below to either call the DCS(Dept. of Child Safety) or a Law Enforcement Agency (Police) • *You need to document who you called and fill out the form and turn it in to your PRINCIPAL with in 24 hours.
