Who is in the multi-professional team • 1. 3 WTE consultants in Palliative Medicine • 2 WTE speciality trainees (registrars) rotate annually • 0. 6 WTE band 8 a CNS • 3. 6 WTE band 7 CNSs • 2 WTE social workers one trust employed predominantly working at Barts, one employed through LBTH but post at risk • 0. 4 WTE consultant clinical psychologist • 0. 6 WTE clinical psychologist • 2 WTE admin support
How do we work • Referrals by phone to main office number 020 34655600 or face to face as staff are on the wards • Most patients are inpatients approximately 750 -800 new referrals a year of which 75% have cancer • One clinic Tuesday morning at The Royal London Hospital • One clinic Thursday afternoon Barts • Clinical psychologists see mainly outpatients
How do we work • Monday to Friday 9 -5 • OOH telephone advice to clinical staff from a consultant on call • Daily meeting to handover new patients, allocate referrals not already seen, discuss patients as needed to refer to other members of the team or arrange joint visits • Triage system through the day • Weekly MDM
Things we do • Advise clinical teams • Support patient and family • Look at how we can support discharge but not completing CHC fast track forms in total • Support referral to hospice and CPCT • Share information through good communication
Improving EOLC • CQC rated inadequate • Separate EOLC work stream and improvement plan • Trust wide steering group SRO is CMO • Hospital site based EOLC groups • Main focus is 5 priorities of care, identifying patients, governance, training • Contributing to TST EOLC work with a view to improving advance care planning, coordination of care, reduce hospital admissions
