Our Rationale for a South Hub • We have a vision for whole system change in West London: • We are committed to the development of personalised, integrated and joined up pathways of care across health and social care; shifting care from acute to community and primary care settings, and reducing hospital admissions and improving early discharge. • Under our current model of care, we cannot afford to meet future demand. Hospital is too often the answer. We need better planned and more proactive care, delivered out of hospital to provide better outcomes for our patients at lower cost. 2

• In West London there is a vision for joined-up good quality Out of Hospital Services with General Practice at the core. 3

Options for Voluntary Sector access and presence • We cannot replicate the model of space allocation applied at St Charles • Provision of services at the hub and colocation • Regular sessional presence for delivery • Pop up space • Visiting for seminar/ support sessions • Signposting to existing service provision • Physical presence

Key questions • How do we embrace the breadth of voluntary sector providers into the service model • How do we generate a wellbeing presence within the hub • Which organisations are best aligned to the services in the hub • Mapping of service providers to potential hubs
