BELAHAN OTAK

Cranial nerves - spinal nerves carry sensory and motor information from the level of the sacrum and coccyc (sacral and coccygeal nerves) to the level of the neck (upper cervical nerves) - above this level, in the region of the head and face, nerves enter the cranium rather than the spinal cord and are termed cranial nerves - there are 12 pairs of cranial nerves and you need only learn them in the context of specific sensory and motor lectures to follow

MEDULLA SPINALIS q Dif: Merupakan bagian SNC yang terdapat di canalis vertebralis q Mulai setinggi for. magnum s/d. setinggi VL 1 -2 conus medullaris q Terdiri atas: 8 segmen cervicales 12 segmen thoracales 5 segmen lumbales filum terminale 5 segmen sacrales 1 segmen coccygeus q Terbagi: a). Subst. alba: fun. anterior, fun. lateralis dan fun. posterior b). Subst. grisea: columna grisea anterior … cornu anterius columna grisea lateralis … cornu laterale columna grisea anterior … cornu posterius canalis centralis

PENAMPANG MELINTANG MEDULLA SPINALIS • BAGIAN LUAR SBG SUBSTANTIA ALBA FUNICULUS ANTERIOR o. k. banyak mengandung FUNICULUS LATERALIS tractus/fasciculus dng FUNICULUS POSTERIOR ser. bermyeline • BAGIAN TENGAH SBG SUBSTANTIA GRISEA, BERBENTUK KUPU-KUPU DNG DI TENGAHNYA ADA CANALIS CENTRALIS COLUMNA GRISEA ANTERIOR cornu anterius COLUMNA GRISEA LATERALIS cornu laterale COLUMNA GRISEA POSTERIOR cornu posterius banyak sel saraf banyak vasa darah ser. tidak bermyeline

Penampang melintang serbagai segmen medulla spinalis

Nuclei di Medulla Spinalis • Terletak di substantia grisea, bergerombol di tempat-tempat tertentu • Di columna grisea posterior: nuclei sensoris, yaitu: di cornu posterius: sel Golgi tipe II dan sel Golgi tipe I (nucleus proprius) di basis columna grisea anterior: nucleus dorsalis (Clarke) • Di cornu laterale: nucleus intermedius (sbg nuclei autonom) - sel-sel autonom sympathis (segmen thoracolumbalis) - sel-sel autonom parasympathis (segmen sacralis 2 -4) • Di cornu anterius: nuclei motoris

ASCENSUS MEDULLORUM • Pengertian: Adalah suatu keadaan seakan-akan medulla spinalis tertarik ke cranial akibat terjadinya intumescentia cervicalis dan intumescentia lumbalis • Intumescentia cervicalis adalah pembesaran medulla spinalis di regio cervicalis akibat pertumbuhan extremitas superior, mulai dari segmen C 3 s/d segmen Th 2, maximum pembesaran pada segmen C 6 • Intumescentia lumbalis adalah pembesaran medulla spinalis di regio lumbalis akibat pertumbuhan extremitas inferior, mulai setinggi segmen Th 10 s/d segmen L 2, maximum pembesaran pada segmen Th 12

N. SPINALIS Ø Adalah saraf yg berpangkal di medulla spinalis, mrpk gabungan serabut-serabut radix anterior dan posterior Ø Serabut-serabut radix anterior dan posterior dlm membentuk n. spinalis hrs melalui for. intervertebrale yg sesuai terlebih dahulu Ø Terdapat 31 pasang nn. spinales, yang masing-masing berhubungan dengan setiap segmen medulla spinalis Ø Setiap n. spinalis mengandung serabut motoris, serabut sensoris dan serabut postganglioner sympathis Ø Akibat terjadinya intumescentia cervicalis dan lumbalis serta adanya perbedaan pertumbuhan antara medulla spinalis dan columna vertebralis, maka serabut-serabut radix anterior dan radix posterior bagian caudal dalam mencapai for. intervertebrale yang sesuai harus berjalan descendens terlebih dahulu; berkas serabut-serabut yg berjalan descendens tersebut akan membentuk: cauda equina

MENINGES SPINALIS v TERDIRI ATAS 2 LAPIS, YAITU: • (1).

ARCUS REFLEX • KOMPONEN: - reseptor - konduktor I ( ser. afferens) - sentrum - konduktor II (ser. efferens) - efektor Serabut-serabut bantu: - ser. associatie/commissura - fasciculus proprius • Reflex: - reflex intrasegmental homolateral/kontralateral - reflex intersegmental homolateral/kontralateral

SERABUT DI MEDULLA SPINALIS v Serabut-serabut yg akhiran, asal dan fungsi sama disebut: Tractus v Serabut-serabut di daerah tertentu disebut: Funiculus q Nama: 1). Disebut berdasarkan asal dan akhirannya Misalnya tractus corticospinalis 2). Disebut berdasarkan lokasinya Misalnya fasciculus dorsolateralis 3). Disebut berdasarkan orang yg pertama kali memperkenalkannya Misalnya fasciculus dorsolateralis tersebut no. 2 disebut dng tractus Lissauer

SERABUT RADIX POSTERIOR • A. BELAHAN MEDIAL - Cabang perifer reseptor - cabang sentral: * cab. ascendens, dlm fun. posterior: nucl. gracilis/cuneatus columna grisea postor di atasnya * columna grisea posterior yg sama * cab. descendens : - fasc. interfascicularis (tract. comma Schultz) - fasc. septomarginalis Homolateral Kontralateral - fasc. gracilis/cuneatus - tract. spinocerebellaris ventralis - tract. spinocerebellaris dorsalis - tract. spinothalamicus anterior - fasc. columna grisea postor atasnya Sensasi: - Raba-tekan - Proprioceptive
