Shoulder Bursitis Inflammation of the bursa Bursa- fluid filled sac located at the top of arm bone and the shoulder Fluid filled sac, works as a cushion and surface for gliding to reduce friction between tissues of the body When the tendons and bursa begin to swell, the space can become too narrow for normal motion. This is what causes the Impingement When movement occurs, the tendons and bursa become pinched between the bones
Causes of Shoulder Bursitis Repetitive/overuse movement Hobbies that put a lot of stress on shoulder Overhead work Repeated throwing activities
Symptoms- Shoulder Bursitis • Shoulder pain when arm is raised • Shoulder joint has reduced range of motion • Redness and swelling around the shoulder • Pain if the shoulder is touched • Pain becomes worse at night
Diagnosis- Shoulder Bursitis Physical examination Radiography MRI Injecting a local anesthetic (i. e. , novacine), into bursa. If pain goes away then most likely pain is coming from there
Treatment/Prevention. Shoulder Bursitis Rest- reducing shoulder movement, shoulder brace can help Anti-inflammatories- aspirin, ibuprofen Icepacks- decrease the size of the blood vessels Physical therapy- strengthening muscles of rotator cuff can decrease impingement of Acromion
Thoracic Outlet Syndrome Due to compression of nerves and/or blood vessels because of inadequate passageway in the neck and shoulder Thoracic outlet- passageway through an area between the base of the neck and armpit Deprive the tendons, ligaments and muscles of oxygen and nutrients
Causes of Thoracic Outlet Syndrome Muscle enlargement (weight lifting) Injuries Extra rib extending from neck Weight gain (obesity) Too much build up of scar tissue Aducted shoulders
Symptoms of Thoracic Outlet Syndrome Numbness in fingers and hands Pain in shoulder, arm, and neck Upper extremity can feel weak Arm going to sleep Discoloration of hand is blueish
Diagnosis of Thoracic Outlet Syndrome Certain maneuvers of the arm and neck that cause pinching and loss of pulse Adson maneuver EAST or Hands up Test Costoclavicular Maneuver Allen Test Provocative Elevation Test Electromyogram (EMG)
Adson Maneuver Positive test = disappearance of pulse
EAST or Hands Up Test Positive test= pain, heaviness, arm weakness or numbness and tingling of hand
Costoclavicular Maneuver Positive Test= absence of pulse shoulders retracted and depressed
Allen Test Positive test= if radial pulse disappears when patients head is rotated
Provocative Elevation Test Positive test= shoulders elevated forward while arms are crossed and experiencing symptoms of numbness or tingling
Treatment/Prevention of Thoracic Outlet Syndrome EXERCISES THAT STRETCH OPEN THE TISSUES OF THORACIC OUTLET ANTI- INFLAMMATORY MEDICATIONS (E. G. , IBUPROFEN, ALEVE ETC. ) SURGERY TO OPEN THE THORACIC OUTLET.
