Introduction - our challenges The population in our area is growing. By 2035, our over 65 s population will have increased by 43% That means increased pressure on the NHS and social care to look after the local community, particularly as it gets older. @CPRCCG www. castlepointandrochfordccg. nhs. uk 3

The journey so far. . – GP Federation weekend GP services, – Innovative community services – Care Co-ordination – Simpler, better GP services in our care homes – Identify & support the most frail in the community – supporting the provision of different types of consultations (e-consultations, telephone consultations) across a range of GP practices – Different types of healthcare professionals available at local GP practices, Pharmacist, Advanced Nurse Practitioner, Emergency Care Practitioner @Southend.
