Serum Creatinine Presented By Assist. Lecturer Aseel Ghassan Daoud M. Sc. In pharmacy/ Clinical laboratory sciences
What is creatinine and creatinine clearance test? • This test measures the level of the waste product creatinine in blood and urine • This test provides more sensitive measure of renal damage than blood urea? What is creatinine? • It is a non protein end product of creatine metabolism • Its amount in serum is proportional to muscle mass
• It is easily excreted by kidneys with minimum or no tubular reabsorption • It is directly related to GFR • Its level remains normally constant Blood creatinine level: Ø It shows how well the kidneys are working Ø It is higher in men than in women
Creatinine clearance test: Ø It measures how well creatinine is removed from the body by kidneys Ø It gives better information than blood creatinine test on how well the kidneys are working Ø It is done on both a blood or urine sample
Reference values: • Serum q Men: 0. 7 -1. 2 mg/dl q Women: 0. 5 -0. 9 mg/dl • Clearance test q Men: 97 -137 ml/min q Women: 88 -128 ml/min
Pathological: • Increase: v Increased rate of formation:
v Diminished renal excretion of creatinine:
• Decrease: v Muscle disease such as muscle dystrophy
What are medicines that increase blood creatinine? • Anti hypertensives: ACE inhibitors ex: captopril • NSAIDs: ibuprofen, naproxen • Diuretics: furosemide • Antibiotics: aminoglycosides (gentamicin) and cephalosporins (cefoxitin)
