Who did we start with? Literally Homeless Households Money Follows the Person (aka exiting an institution)
Houston Housing Authority Admin Plan Each year HHA gives a preference to no more than 200 applicant households meeting all of the following criteria: 1. Meet the federal definition of homeless; 2. Are referred to HHA by a homeless service provider with whom HHA has executed a Memorandum of Understanding (MOU) outlining the provider’s responsibilities with respect to the provision of housing search assistance and supportive services for the referred household;
Houston Housing Authority Admin Plan (cont. ) 3. Have received a written commitment from the referring homeless service provider for housing search assistance; 4. Have received a written commitment from the homeless service provider for supportive services to help the household’s transition from homelessness to permanent housing; and 5. Have received a written commitment from the homeless service provider for supportive services to help the household comply with Housing Choice Voucher program rules.

The Process (cont. ) Coalition Trained Houston Center for Independent Living in HMIS Role: Executed Data Sharing Agreements with all Partners MCOs working through their legal so that they can enter into HMIS Goal is to have HCIL set as Coordinated Access Hub
The Process (cont. ) Initial Client List: HCIL had existing waiting list for NED vouchers Common clients from this list and MCOs were pulled Data match between HMIS and MCOs to also identify common clients Priority is driven by HHA preference or vulnerability score if available
Results Single point of contact for HHA Briefings twice per month HCIL is gatekeeper Effective date 1/1/19, 99 vouchers, 78 issued, 20 leased
