Session Objectives • Connect with other FORHP RHND grantees to identify opportunities for peer learning that can be explored over the conference • Gather advice and resources from peers to take home with you • Share your own successes and tips • Create opportunities for future connections

Table Discussions - Consider switching seats to maximize networking

Getting Connected Introductions at your Table (under a minute each) • Name • City/state and organization • Brief network description and focus area of work funded by the current RHND grant • A fun fact about your community

“Celebrations, Reflections, and Sustainability” • Opportunity to network and make peer connections • Reflect and celebrate successes to date, share lessons learned, and discuss sustainability • Opportunity to gain insights from peers with a range of expertise and perspectives

Peer Interaction: Instructions I. Identify a table Facilitator/Timekeeper and a Reporter II. Reflect on your answers to the following questions, taking notes as needed to report out. 1. What are you most proud of achieving at this point in your RHND grant initiative? 2. What’s been a key learning, insight, or discovery from doing this work? o 3. Complete the following - “If there was one thing I would share with others about this work, it would be …. ” What are the primary strategies you are pursuing to achieve sustainability of your Network grant initiative?

Peer Interaction: Instructions III. Discuss as a group, 1 question at a time o Facilitator keeps time (~10 minutes per question) IV. Reporter – Briefly share a top lesson learned & sustainability strategy from discussion

Next Activity Set Up and Break • Following our 15 -minute break…you will choose a new table when you return. You will select a table that reflects one of the network characteristics that you chose to include in your sustainability plan. Something that you might like the insight of your peers to address… (If you need a copy of your sustainability plan as a refresher, please let us know during the break. )

Peer Partnering for Sustainability Each table will need a facilitator/timekeeper and a recorder. Each person will need a discussion guide handout.

Next Steps for Peer Groups • A TA Provider will be assigned to support each peer group. • The TA Provider will contact the listed peer volunteer for the group to debrief and discuss next steps. • TA Provider will support the logistics of setting up and communicating next interaction.
