DEFINITION “ Running commentary are narrative descriptions for some incidents which describe behavior, personality traits, presence or absence of skills and understanding”
PURPOSES o o o It serves as a basis for evaluation. It serves as an aid to memory. Creates awareness of problems. Allows for practice of decision making It serves as an aid to clarify statements and issues.
POINTS TO BE INCLUDEDIN IN RUNNING o o o o COMMENTRY RECORD It is skills in assessing and meeting patients needs. Ability for planning, organizing and controlling. Ability to motivate the staff. Safety measures for patients. Assuring professional responsibility. Effort in self development. Developing and training the employees.

POINTS TO BE INCLUDEDIN IN RUNNING COMMENTRY RECORD o Record should include the personal data of the head nurse. o Statement should be based on planned or incidental observation only. o Statement should be recorded as soon s possible after the incident, before the details have blurred.
GUIDELINES FOR RUNNING COMMENTRY RECORD o Each note should be written with date and time, sequence of date may be informative to recall the incident , when the entire records are reviewed. o Statements should include what was heard or seen only, not based on opinion. o Comments that have to be recorded should be described as you have heard in closed in…
GUIDELINES FOR RUNNING COMMENTRY RECORD o Record should have sufficient number of anecdotes (or narratives) to determine the characteristic behavior of the individual. o All statements should be signed for the person who writes and by the person who is written about. o Observations should be recorded at least once in two weeks and when ever incidents occur.
GUIDELINES FOR RUNNING COMMENTRY RECORD o Statement should be clear, concise, meaningful and up to date. o Periodic conference conducted to discuss quality of work. o The running commentary record is valid for one year only. “ forget about the past year narratives”
GUIDELINES FOR RUNNING COMMENTRY RECORD o It should contain assures for the following: § Who did and who said § What was done or said § When did it happen
GUIDELINES FOR RUNNING COMMENTRY RECORD o Statement should include both positive as well as negative attribute to the person. § Positive and constructive points should be emphasized. § Weak points should be discussed without delay for guidance.
GUIDELINES FOR RUNNING COMMENTRY RECORD o The second in-charge is instructed to write the incidence during the HN absence in a piece of paper signed by the nurse and second in-charge to be over to the HN to fix it in he file. o The CI are requested to follow the previous points , but with the presence or absence of the HN.
Example 1: Running commentary Positive points: 10/5/2017 At 8: 00 am during bed side round, I observed S/N (Jency Shoby) doing I/V cannula insertion for keep vein open to her assigned patient in room 2 Bed 7, diagnosed gallbladder stone. The S/N checked all equipment needed for the procedure, explained the procedure, hand washing. The S/N (Jency Shoby) inserted the cannula in first attempt and following sterile technique during the procedure. The cannula with sterile plaster noted the time and date of insertion at the cannula site. After the procedure the nurse disposes the sharps in yellow sharp box and washed her hand. The head nurses checked the nurses note which she had documented as time, position of the I/V cannula with her signature. I appreciated the staff nurse for her performance and patient care during the procedure. Staff Nurses Head nurse
Example 2 : 10/5/2017 At 7: 00 am during attending the endorsement and all the staff nurses attended on time with no interruption, the group leader (Rincy john) endorsed basic identifying data, changes in doctors order and patients respond in loud voice enough for all to hear, all seated with paper and pen, all patients covered. I gave feedback and encouraged the staff to help each other's and to provide quality of care. I appreciated her following endorsement guidelines of MOH and encourage her keep on up. Example : 3 Negative Points: 10/5/2017 During bed side round at 8: 00 am, I smelled bad odor from the bathroom, the floor was wet. When patient's room checked, I observed that the environment around the patient was so dirty and the garbage was full of waste in R 5 Bed 22. This patient assigned to S/N ( Laila Ali), I explained to the staff nurses about her responsibility as measure safety environment according to job description of staff nurses guidelines of MOH. Example: 4 10/5/2017 During my round at 12: 30 am, I observed one of patients mother for Room 2 Bed 7 , Egyptian diagnosed Hemorridectomy. The mother complained about her daughter fasting (NPO) from 8: 00 am and staff nurse didn’t informed her operation time. She said " I don’t know for how long I will wait? the operation is cancelled or not? " She asked the group leader Susan Koshy, but she didn't communicate effectively with her and she said "I will inform you later". I communicate with mother and let her to express her feeling and told her " operation room will call if they ready to receive your daughter don’t worry". I discuss with the G/L (Susan Koshy) about the principle of effective communication according to MOH and she accept it. Staff Nurses Head nurse
