Resources, Tools and Templates Support material Team Around the Learner recognises the need to support collaborative practice by ensuring high levels of transparency and documentation as the team move through the Team Around the Learner approach. A number of resources are provided to support this approach. It is also evident that different teams may at times wish to access further information from the Department of Education & Training. A series of links is also provided. These resources are intended to: • be available to all members of the team that are working together • support greater clarity across the team of the learners goals and challenges • enhance the team to work more effectively and efficiently together • support shared communication across the team What is provided The following provides an alphabetical index that highlights each tool. 2

Resources, Tools and Templates Tool/Template Purpose TAL phase most relevant Referenced in Modules Fact Sheet An overview of the Team Around the Learner approach and the resources and modules available to support Initial Needs Identification Tool A tool to facilitate the collection of a wide range of data and information about the learner that will inform the plan. Initiate Contact Analyse Needs 7 – Initiate Contact 8 – Analyse Needs Learner Profile A template to record a learning profile and identify the strengths and the barriers impacting the learner; current and suggested reasonable adjustments and strategies. Information collected here will assist in creating meaningful goals and plans. Analyse Needs 6 – How does the team share information 8 – Analyse Needs Meeting Agenda/Minutes template A template to plan and record the purpose, approach and desired outcomes of the meeting. To also record all discussions and outcomes as minutes of the meeting Monitor & Evaluate 4 – Role of the Lead Professional 5 – How does the team share information 10 – Monitor & Evaluate 11 – Planning Transitions Planning Improvement Tool This tool supports schools to put improvement plans in place for any phase of the Team Around the Learner approach which is identified as needing further development. Resources, Tools and Templates Tool/Template Purpose Reflective Practice A guide to assist teams to reflect of their actions to determine effectiveness and how things may be done better in the future Rubric To assist schools to measure their practices related to the Team Around the Learner approach. Used in conjunction with the Planning Improvement Tool Transition from Additional Support A document recording how the transition from additional support will occur, including any future needs and planned follow-ups.
