Example #1 �Caregivers were told by supervisor to wake up all residents by 4: 30 am for showering and have them dressed and moved into common area by 5: 30 am.

Example # 2 �Staff placed a resident in a mechanical lift chair in a reclined position and then unplugged the chair to keep the resident from getting up on her own. The resident was found on the floor, in front of the chair, with head injuries requiring nearly two weeks in the hospital.

Example # 3 �A resident was observed taking a bath with the bathroom door open, in view of other residents.

Example #4 �A caregiver took a photo of a resident seated on the toilet and uploaded the photo onto the caregivers social media page. The caregiver stated she wanted her friends to see what she “had to put up with” at work.

Coercion and Retaliation – DHS 89. 36 �“No person may engage in any form of coercion to prevent a tenant, employee or service provider from filing a complaint about a RCAC or any form of retaliation against a tenant, employee or service provider for filing a complaint. ”
