Ensuring data quality prior to reporting FACILITY RESPONSIBILITY: § Facilities must run and action the line lists in TIER. Net prior to monthly / quarterly reporting, to ensure: o Defaulting patients are traced and recalled. o Clinicians take action for all patients on the lists, so that all lists are ‘zeroed-out’. o Capturing backlogs are addressed. o Data is cleaned. § Facility Manager is responsible for ensuring the above is completed. This applies to all TB and HIV reporting.

Producing monthly TB Case Identification report from TIER. Net FACILITY RESPONSIBILITY: § § § Generate the TB Case Identification Report from TIER. Net. Transcribe monthly data from TIER. Net TB Case Identification Report onto the Monthly Data Input (MDI) form. Facility Manager to verify and sign off the MDI form. Store a copy of the signed-off MDI form as well as the TIER. Net report in the facility binder. Submit the signed-off MDI form to Sub-District, by the 5 th of each month.

Producing monthly report from TB Case Identification register FACILITY RESPONSIBILITY: § § § Summarise all reportable data into the Data Summary Sheet (at the end of each register). Summary Sheets remain in the facility for audit purposes. Transcribe monthly data from Summary Sheet onto the Monthly Data Input (MDI) form. Facility Manager to verify and sign off the MDI form. Store a copy of the signed-off MDI form in the facility binder. Submit the signed-off MDI form to Sub-District, by the 5 th day of each month.
