Registers remain in the facility for audit purposes. Transcribe monthly data onto the Monthly Data Input (MDI) form. Facility Manager to verify and sign off the MDI form. Store a copy of the signed-off MDI form in the facility binder. Submit the signed-off MDI form to Sub-District, by the 5 th day of each month.
