What to report? Just like talking to your family… - Not only bad things are reported - It’s a constant flow of information - You encourage actions to repeat good behaviour - You aim to prevent accidents to happen by reporting things that could have gone wrong or did go wrong - deed W T

What to report? Not only bad things are reported - Calling your family to say that the trip went fine and that you landed safely back in the Philippines(with gifts…) - Reporting to the CCR that you hear that the cargo is flowing at the Manifold and no leaks have been noted when checked, (asking for a hot cup of coffee…) - deed W T

What to report? It´s a constant flow of information - Family gathering with the relatives at home sharing a good meal discussing the upcoming graduation of your son - Sitting in the dirty mess with the rest of the crew at coffee break eating sky flakes and discussing the tank cleaning you will be doing the next day - deed W T

You encourage actions to repeat good behaviour - - You thank gracefully your spouse that he/she let you sleep last night when he/she was up with the crying baby so you could sleep - You thanks gracefully your college that he woke you up and told you that the coffee was already prepared in the dirty mess for you to wake up before coming on watch deed W T

Barriers. . Why is no everything reported? - Feeling that you will get the blame in case you report a personal mistake - Thinking that small mistakes or incidents does not matter as they were minor and no harm done - Believing that it was human error and therefore it can not be prevented - The norm is that it normally happens close-calls and minor incidents and therefore it is normal when it occurs again - There is no feeling that the reports are being followed up correctly - deed W T
