CLINICAL CASE 20 y woman with right red and painful eye and visual change after ten days of progression. Ø Visual acuity: 1 both eyes Ø BMC: dilated scleral vessels without whitening after fenilefrine and no Tyndall Ø Funduscopy: nasal optic disc edema, vascular tortuosity, macular star iniciation without vitritis or vasculitis
RESULTS Ø B-scan ultrasonography: T-sign Ø Angiography: macular folds. optic disc edema and
RESULTS Ø Macular OCT: macular folds and thickening associated to two neurosensorial detachment. Ø Optic nerve OCT: Optic disc edema. DIAGNOSIS: posterior scleritis TREATMENT: prednisone 60 mg
EVOLUTION ØFollowed by Rheumatology and Ophthalmology, Orbitary TC, orbitary RMN with and without contrast, thorax Rx: normal o Serology and antibody studies: negative ØTen days after the event, macular OCT is normal, so the corticosteroid dose was gradually tapered off during three months. o ØNEW EVENT a month after finishing therapy: prednisone 60 mg, methotrexate, folic acid, vitamin D and calcium. 457µm
EVOLUTION ØCurrently, after five months with therapy, she persists asymptomatic with normal OCT and B-scan.
