Completion of Clinic Cards Students are encouraged to complete the information outlined by the red oval. • Each student, at a minimum will have one Clinic Card completed per week (e. g. , a student assigned to wards). • Each student, requires a Clinic Card for each separate clinic to which he/she is assigned. St. Clares X Jill Student Jean Resident 15/09/24 • For a given Clinic Card, a supervisor/preceptor is required to comment on at least one EPA, but no more than 4 EPAs. • Clinic Cards covering a week long period would likely cover more EPAs than a Clinic Card reporting on a half-day clinic.

The student will be monitoring his/her assessments to ensure that all required EPAs are being assessed adequately.
