Quality Improvement Project PDSA Summary 2019 Paediatric Wheelchair Referral Pathway - Calum Maciver Aim: To promote joint working and multiagency assessment by formulating a revised standard operating procedure for: - Paediatric wheelchair referral pathway - Wheelchair Review form Objective: By enabling more efficient professional liaison and consultation between Birnie AHP’s and Wheelchair Services, this will promote a patient focused service that supports the individual needs for new and existing wheelchair users. Measure(s): Birnie Centre AHP Questionnaire sent to 5 therapists (PT & OT) to test effectiveness of referral pathway and review form including areas requiring further development. Result: Changes now part of mainstream practice for Highland wide team. Ongoing Information gathered will be feedback during wheelchair review meeting to plan next step. Cycle 6: Update OT/PT teams of implementation of paediatric pathway and review form into mainstream practice. Cycle 5: Gather data - 2 therapist unable to implement due to change in circumstance Review meeting November 2018 to discuss key messages and finalise change ideas. Cycle 4: Discussed changes to procedure with participating therapists and agree timeframe for test of change. Cycle 3: Identified need for change to evaluation process to include questionnaire on effectiveness of changes following period of testing. Change Ideas: Create wheelchair review form for existing users Adapt wheelchair referral pathway to enable objectives Cycle 2: Adapt standard operating procedure to include new wheelchair review form to accommodate existing users and agree new layout of pathway with wheelchair services. Cycle 1: Review current pathway with wheelchair services and identify key areas for more effective joint working. Update plan to establish roles and procedures including status of wheelchair user.
