Primary Contact • Your contact information will go on all recruitment materials and program information for families § You may consider making a generic BHF@company. org email to use for all communication • Be sure to reach out to other organizations in your community to help. Having 2 -4 volunteers (in addition to your content coordinators) assist each week will make your responsibilities MUCH easier.

Recruitment • Recruiting families that qualify for the program is essential for implementing BHF.

Recruitment • Recruiting families that qualify for the program is essential for implementing BHF. • We have created a module specifically for recruitment and provided templates for: § Physician Referral letters § Fliers, brochures § Newspaper ads • As part of implementing BHF, you have access to use the BMI Reporter Web application in your local school districts to identify and encourage families to participate.

Information Meeting Interested families should be encouraged to attend an informational meeting. This meeting

During this time you will collect: § Family consent to participate § Family contact Information § Baseline data and determine eligibility • You will want your coordinators and volunteers to attend the information meeting to assist you in gathering the information!

Eligibility • To determine if a family qualifies for the program, they must have

Eligibility • To determine if a family qualifies for the program, they must have at least one child with a BMI percentile ≥ 95 th. • In the data portal, you will find the CDC BMI percentile calculator to use for the children and their siblings. You will need date of birth, height and body weight. § Children 85 th- 94. 9 th percentile may still participate in the program, although weight maintenance will be the goal instead of weight loss.

Scale and Height Stadiometer • As part of the start-up funds, your organization will need to purchase: § 2 digital platform scales that measure to the tenth of a pound for accurate measurement of body weight. § 1 -2 portable height stadiometers that reads to the nearest 1/8 th inch. • Accurate measures of body weight and height are essential for eligibility measures and outcome measures. • A computer (or two) will be required for weekly data entry. An i. Pad, laptop or desktop computer with a printer will be ideal for weekly weigh-ins at each station.

Family Binder • After the families sign up for the program at the information

Family Binder • After the families sign up for the program at the information meeting, but before the first session, you will put together a binder to give each family. • Each week, the family is asked to bring the binder to the session. § Weekly education materials are handed out during the education week, not prior.

Weekly Education • Contacting families § Through the data portal, you can send registered family members emails or text message reminders of meetings and messages. • BHF is an interactive education program. Each week, to deliver the education components, it is important that families are actively engaged. Therefore, supplies and equipment are necessary to deliver the program.

Weekly Supplies and BHF Kit • Each week, you will want to review the weekly lesson plans to obtain a list of required supplies for the week. In addition, we like to send home resources for families to use. For example, during the portion size education, you will give each family a set of measuring cups. These will be used for education and they can take them home.

Your program will need to partner with either a PE teacher or facility that has activity equipment for children (YMCA). § Unique items have been included in your kit. Some items will need to be purchased – i. e. Spikeball games. Be sure to read lesson plans in advance so you can order your supplies and equipment.

Each module lesson plan has a detailed list for every education session. Be sure to follow the lesson plan!

Physical Activity Monitors • You will need to purchase activity monitors for the child

Physical Activity Monitors • You will need to purchase activity monitors for the child participants in your program. We highly encourage everyone in the family to wear one. They are great motivators and help keep each other motivated. Competition is good! • You will need to determine if you want to purchase wearable physical activity monitors for parents as well, but due to cost, you may just provide the participant with one and encourage the families to purchase their own.

Organization/Community Volunteers • You may be able to identify others within your organization or community who want to help deliver BHF but are not a “coordinator”. • Assign these people to help with: § Food or Activity preparation before BHF § Weigh-in, collection of habit books and physical activity data § Distribute weekly hand-outs § Assist with demonstrations/activities § Engage with families § Help at the physical activity station
