Primary skin lesions
1 - Macule: Flat, circumscribed area of skin discoloration <1 cm Example : freckles , lentigo
2 -Patch: Flat, circumscribed area discoloration >1 of skin cm Example : melasma , vitilligo
3 -Papule: Circumscribed elevation of the skin < 0. 5 cm in diameter Example : Molluscum Contagiosum , lichen planus, common warts
4 - Nodule: Circumscribed visible or palpable lump, > 0. 5 cm Example : erythema nodosum , boil , lipoma
5 -Comedone : Clogged hair follicle (pore) pin head size in the skin. A comedo can be open (blackhead) or closed by skin (whitehead) Example : acne vulgaris
6 - Plaque: Circumscribed, disc shape , elevated area of skin: ‘ small ’ < 2 cm in diameter ‘ large ’ > 2 cm in diameter Diameter > depth Example : psoriasis , pytriasis rosea
7 - Vesicle: • Small visible collection of fluid ( ≤ 0. 5 cm in diameter) • Example : herps simplex , varicella ( herps zoster and chicken pox ) , pompholyx
8 -Bulla: • Large visible collection of fluid ( ≥ 0. 5 cm) • Example : bollous dermatosis (pemphigus vulgaris >> flaccid , and pemphigoid>> tense)
9 - Pustule: • Visible accumulation of pus • Example : folliculitis , acne vulgaris , pustular psoriasis
10 -Wheal: • Circumscribed, elevated area of cutaneous erythematous edema (transient) • Example : urticaria
11 -Burrow: • Slightly elevated , grayish , tortuous line in the skin ended by papule. • Example : scabies
secondary skin lesions
1 - Scale: • Visible and palpable flakes due to aggregation and/or abnormalities of shed epidermal cells (thick stratum corneum ) • Example : psoriasis , icthyosis , lichen plannus
2 -Crust: • Accumulated dried exudate, e. g. serum • Example : impetigo , ecthyma
3 -Lichenification: • Flat - topped thickening of the skin often secondary to scratching • Example : chronic eczema
4 - Excoriation (abrasion) : • Secondary, superficial ulceration or erosion, due to scratching • Example : eczema
5 -Maceration: Appearance of surface softening due to constant wetting
6 -Umbilication: • Depression at the top of a papule , visicle or pustule • Example : Molluscum contagiosum
7 -Ulcer: • Loss of epidermis (often with loss of underlying dermis and subcutis as well) • Example : chancre
