Mishap/Near-Miss/Unsafe Reporting Procedures Mishap - is any unplanned or unexpected event. n Near-Miss - is any situation which allowed to go unchecked or uncorrected has the potential to cause a mishap. n Unsafe Reporting Procedures - are the responsibility of all DOD personnel. n
