PRACTICUM IN HEALTH SCIENCE PORTFOLIO
PORTFOLIO BINDERS • You may use a 3 -ring binder (1” to 1½” will be fine) • It should have a clear cover so that you can insert a cover page • You should insert a page on the spine of the binder with your name on it as well. • Make sure you use the SAME FONT throughout the portfolio unless you have some artistic dividers or cover pages.

EXAMPLE OF PORTFOLIO TABLE OF CONTENTS • Each item matches tab on notebook dividers • The dividers should have tabs with numbers or plastic inserts to label the sections.
EXAMPLES OF PORTFOLIO DIVIDER PAGES • Each section should have a page that describes what is in that section. • Sometimes called divider pages.
FIRST SECTION • Technical skill competencies • These are the skills that are critical to your development as a health care provider • List these skills with a brief description • Include any documents (CNA skills checklist, etc…) that are proof of mastery of skills (you may use plastic page covers to hold smaller documents)
SECOND SECTION • Licensures or certifications: these include (but are not limited to): • CPR • Certified Nursing Assistant Certification • Registration as a Pharmacy Technician Trainee
THIRD SECTION • Awards and scholarships • This section is reserved for special recognitions that you have received. Examples include: • Academic honors • Awards such as Eagle Scout • Scholarships based on academic achievements
FOURTH SECTION • Activities: this section should include ALL of these: • Community service: these are OUTSIDE OF SCHOOL activities • Volunteering at Food Bank • Church volunteerism • Participation in HOSA (leadership or competition) • Any other technical or professional organizations • Under each activity you need to describe what you did for that activity.
FIFTH SECTION • Samples of work • Include anything you think demonstrates professional growth • You must include all of your internship reports • These should be formatted for ease of reading • Be sure to do a good job on them throughout the year. Print them at the time you complete them and place them in portfolio to save you time.
SIXTH SECTION • Resume • Should include items listed on rubric • 1 -2 pages in length
SEVENTH SECTION • Evaluations • Include all evaluations from your Walgreens externship and your fall internship
PORTFOLIO WALK • End of the Spring Semester • Will have administrators, parents, faculty, internship sponsors, etc… come through the lecture hall to ask questions about your experience in the Health Science Academy. • You are describing everything you have done in the four years! • You will dress up, stand behind your portfolio, and speak professionally
IN CONCLUSION • It shouldn’t feel overwhelming to you • We will work on it gradually throughout the year • There will be check-points of when certain items are due
