Practice Engagement Plan KEY COMPONENTS Identifying which practices and when to engage Staffing your Practice Facilitation Program ü Determine practice facilitator FTE needs (based on recommended ratio of FTE to # of practices). ü Formalize practice facilitator job description: new dedicated role or additional responsibilities to existing roles. ü Identify a practice facilitation program manager and/or lead facilitator. ü Define key objectives and milestones for initial practice engagement and facilitators. Completing Practice Assessment Tool Baselines 1/7/2022 ü Identify the practice unique identifier and calculate number of practices. ü Determine practice engagement model: • Phased: Define criteria to determine readiness to engage and review practices for prioritization. • Continuous ü Establish a schedule detailing by month which practices will be engaged with an assigned practice facilitator. ü Determine primary use of PAT Baseline: to identify readiness to engage and/or to engage practices. ü Describe PAT Baseline and Re-assessment completion methods. ü Calculate monthly projections through end of year in order to complete all baselines. ü Describe the intended PAT process management.
