Practice Engagement Plan KEY COMPONENTS Identifying which practices and when to engage Staffing your Practice Facilitation Program ü Determine practice facilitator FTE needs (based on recommended ratio of FTE to # of practices). ü Formalize practice facilitator job description: new dedicated role or additional responsibilities to existing roles. ü Identify a practice facilitation program manager and/or lead facilitator. ü Define key objectives and milestones for initial practice engagement and facilitators.
