FESHM Updates FESHM Chapter 2060 – Work Planning and Control Updated scope in Rewrite of 2060, incorporating SHAPE into work planning. Identified roles and responsibilities for Authorizing Supervisor, work planner, Point of Contact, and worker. Updated risk matrix and work flow chart. Added additional appendixes. FESHM Chapter 10200 – Lift Plans Initial release of Chapter 10200 FESHM Chapter 4130 – Personal Protective Equipment (PPE) Five-year review of chapter 4130. Section 2. 0 Hazard Assessment and Certification statement revised to reflect current work planning procedures. Training updated from ESH&Q Section’s Safety & Environmental Protection Group to ES&H Section 12. 0 Appendix I PPE training #FN 000199 reference deleted. This training is given during New Employee Orientation. Added the hierarchy of controls to the introduction. Work Planning • Please start to use the IMPACT database for HAs if you have gone through training. – There is a test database to get familiarized with the process and workflow, and personalized training is available. • Remember, under COVID-19 conditions: – HA not needed if it is currently not required by FESHM 2060 and social distancing can be maintained. – HA needed if required by FESHM 2060 under normal circumstances OR for any work that doesn’t normally require a written HA but will now require exposure control equipment (face shields, face coverings, gloves, etc. ) because of close proximity to another worker. ES&H Requirements for Service Subcontractors p 2 • An “Exhibit A For Supply and/or Services Subcontracts” is required for all Service Contracts involving on-site work and must be attached to the requisition or Procurement will not process the req. The Exhibit A form is found here: https: //esh-docdb. fnal. gov/cgibin/Show. Document? docid=1738 – The person who completes the Exhibit A and whose name should appear on the front page is either: • The requisitioner, or a designated point of contact (POC), if the work is all low hazard (see definition in Section 3. 0 of FESHM 7020) or • A trained Service Coordinator (SC), for things that fall into the High Hazard category. There is a table in FESHM 7020 defining what High Hazard work is for this purpose. • Who designates the SC, if needed, is left up to the requisitioner in the chapter, but we expect the requisitioner would contact their department head for high hazard work, so they can assist in identifying the SC. – Contact a DSO if you need help in determining if something is Low or High Hazard after reviewing the definitions in FESHM 7020. 8 Raymond Lewis & Katie Swanson | Department Heads Meeting 1/6/2022
