Clarity of Purpose Set out clearly what you are hoping to change
Definitions and Language Take time to define what you mean by social prescribing and why you think your approach will bring about the changes you want: look around and learn from others.
Monitoring and evaluation Be clear about what impacts and outcomes you want to achieve and how you will monitor and measure changes: be realistic and design-in experimentation.
Collaboration and trust Design and invest in your collaboration and build your model together. You can only progress at the speed of trust Start small and scale up when you have evidence that your process works for everyone.
System Leadership Invest in System Leadership: do not assume that everyone will have the same agenda.
Cost and sustainability Be realistic about what your early pilot/start-up will cost: Once you are clear about critical success factors you can start to embed and “roll-In” elements into business as usual. Use your collaboration for innovative funding sources: it will bind your collaboration Wellbeing Exeter roll out is 30% cheaper than in the design and pilot phase Multiple funding streams Added value through the system leadership model
