Confirm requirements in respect of Permit Request, Inductions, Pre-Task Safety Debrief, Task Specific RAMS & Equipment Certification Safety – A task specific RAMS may be requested for review and comment prior to the day of activity. If RAMS have been requested but are not presented until the day of the activity NO work is to be authorised until the RAMS has been reviewed and accepted as suitable Once all documentation is reviewed and accepted an initial pre-task risk assessment must be performed of the workplace to record all potential hazards and identify control measures Safety – The pre-task risk assessment will identify any existing and/or potential hazards and potential changes to atmospheric conditions. The potential for flammable or toxic vapours must also be assessed. The contractor should be with you during this assessment The permit issuer will ensure that the work area is prepared for the proposed works referencing the requirements specified within the permit and associated documentation i. e. RAMS Safety – Careful attention must be given to ensure all necessary preparation is completed to allow work to be conducted in a safe environment. Isolations, barricading, draining, gas testing, placement of fire fighting equipment and any other preparation should be recorded where applicable Gas test the work area and surrounding area (15 m) for LEL & Oxygen. A competent person will be required to conduct the test and record the results within the section provided on the permit Safety – A calibrated unit must be recorded and used to perform testing activities. If the LEL reading is greater than 0% work must not be authorised. The source of the flammable vapour must be determined and remedial action taken to eliminate it The permit issuer will determine the requirement for continuous monitoring during the activity. The issuer will explain the expectations to the contractor in the event of LEL escalation Safety – Continuous monitoring may be required. If at any time during the activity the LEL rises to above 0% the contractor must be aware that they will stop work immediately and contact the Issuer. Permit to Work Issue (Hot Work) 7 8 9 10 11 12 Reference #: Issued by: Date: Pages: On completion of all documentation the permit issuer will authorise the permit holder to start work. Work must be started within 30 minutes of the permit being issued Safety – Once signed by both issuer and holder the work is authorised to commence. If the work does not start within 30 minutes of the permit being authorised the permit must be returned to the issuer for re-validation prior to any work being initiated The hot work activity must have a suitably trained and competent Fire Watcher identified to monitor all hot works. The fire watcher must be aware of the expectations as laid out in section H 4. 3 Safety – The permit issuer should instruct the fire watcher of their duties as stated in the documented procedure. The fire watcher must not perform any other duty and remain in attendance whenever hot work is taking place. They must have a clear line of sight of the activity Provided that a trained and competent permit issuer remains on site the permit can be extended for a maximum of 16 hours on a daily basis and renewed for up to 6 consecutive days Safety – The issuer must state the date, start time & expiration time on the permit. The permit can then be extended and/or renewed as necessary. The work area must be inspected prior to work starting if there has been any break in the activity to re-validate the pre-task risk assessment For each extension or renewal the permit issuer must confirm all restrictions, conditions and controls remain the same, gas test results are within acceptable limits and the permit is up-dated Safety – This can only be done by visual inspection of the work area. Existing and/or potential hazards may change with time therefore re-inspection is vital. The permit must not exceed the daily time limit or exceed 6 days in concession 2 of 2 Identify Authorised Work A work permit will be withdrawn if conditions arise as stated in H 4. 3. The permit holder will stop work and return the permit to the issuer Safety – Should any of these conditions arise the work must be stopped immediately and the permit returned to the issuer. The permit issuer will then make an assessment and decide if/when the work can safely continue and the permit can be re-issued Under certain other conditions the work permit will need to be cancelled. These conditions are detailed within section H 4. 3 Safety – Should any of the stated conditions arise the work permit will be cancelled. Once documentation and work site conditions have been re-validated the permit issuer can issue a new permit to the contractor 13 WI. 025. 10 Stuart Lynch 31/12/2010 On completion of work and hand-back of the permit issuer must perform a visual inspection of the area to ensure it has been left safe, clean & tidy and the work is completed to requirements Safety – The permit issuer should make an assessment of the work location to ensure it has been left safe, clean & tidy. All tools/equipment should be removed from the work area. An evaluation should also be made of the completed work to Site Alarm / ESD – Stop Work 13 – Permit Sign Off
