Permanent Disability Examples Example 1 v The student requires modified duties on a permanent basis. They should not lift more than 10 lbs and should avoid bending, twisting and squatting. Example 2 v The student was born in 1996. Complete document from 2001 and incomplete document from 2004. The diagnosis is ADHD. The student was 5 and 8 years old.
Permanent Disability Examples Example 3 v Document was Psychological Summary. The only test used for the assessment was the Weschler Adult Intelligence Scale. Example 4 v Diagnosis of MS. The doctor indicates the student has no problems and there are none expected for attending school/studying.
Permanent Disability Examples Example 5 Example 6 v The psychological report diagnosed Test Anxiety and features of Generalized Anxiety Disorder. v The student has been diagnosed with ‘dry eye syndrome. ’ This is made worse by use of a computer. Accordingly examination on paper would be preferred. The student may need an increased time allotment to allow for eye rest and instillation of artificial tears.
Permanent Disability Examples Example 7 v This letter is to confirm the student has ADHD. This has of course made it difficult for the student in certain learning situations. It would be of great help if the student could be accommodated with more time for written exams. Example 8 v Psychoeducational assessment received (dated 3 years ago), but no diagnosis was provided. There is a comment on the previous diagnosis, but it does not confirm a diagnosis in this report.
Permanent Disability Examples Example 9 Example 10 v Emotional Behavior Disorder with Alberta Education. v Psychoeducational report from 1993 when the student was 7. There is no diagnosis in this report. Comments were made on the Schedule 4 that the student has been approved for an updated assessment.
