PERIODONTAL DISEASE HORIZONTAL AND VERTICAL BONE LOSS LOCALIZED
PERIODONTAL DISEASE HORIZONTAL AND VERTICAL BONE LOSS; LOCALIZED & GENERALIZED, MODERATE & ADVANCED (severe) PRESENTED BY: SOVEZA IFTIKHAR , MARIAMA BERETEH DEN 1218 SECTION: D 230
Periodontal diseases - Periodontal means around the tooth - Periodontal disease refers Group of diseases that affects the tissues around the tooth - They are set of conditions characterised by inflammatory host response in periodontal tissues may lead to localized or generalized alterations in the soft tissues and bone ultimately loss of the tooth - Periodontal diseases are classified into: Gingival diseases and Periodontal diseases
WHAT IS PERIODONTITIS? • Untreated gingivitis can advance to periodontitis. With time, plaque can spread and grow below the gum line. Gums separate from the teeth, forming pockets (spaces between the teeth and gums) that become infected. As the disease progresses, the pockets deepen, and more gum tissue and bone are destroyed. This destruction could be in horizontal or vertical pattern. This destructive process has very mild symptoms. Eventually, teeth can become loose and may have to be removed.
• Pattern described as horizontal or vertical. CEJ of adjacent teeth can be used as a plane of reference in determining the bone loss present. HORIZONTAL BONE LOSS Horizontal bone loss refers to the loss in height of the crestal bone around the teeth. ! Horizontal bone loss may be: Mild , Moderate , Severe , Crest remains generally horizontal.
-Most common pattern of bone loss -Bone is reduced in such a way that the bone margin is approximately perpendicular to the teeth surface -Interdental septa and facial and lingual plates of bone are affected, but necessarily to an equal degree around the same tooth
VERTICAL BONE LOSS • Usually localized to one or two teeth. May be several areas of vertical bone loss throughout the mouth • Occur in oblique direction
Vertical Bone Loss is Less common pattern It occurs when the pathway of inflammation travels directly in the pdl space. This type of bone loss produce infrabony pocket. It occurs interdentally It increase with age
BONE LOSS CAN BE DESCRIBED AS LOCALIZED OR GENERALIZED DEPENDING ON THE AREAS INVOLVED. LOCALIZED IS LESS THAN 30% GENERALIZED IS MORE THAN 30% MODERATE BONE LOSS = 3 MM TO 4 MM FROM CEJ APICALLY ADVANCED (SEVERE) =5 MM OR GREATER FROM CEJ TO APICALLY Localized Moderate Periodontitis Generalized Advanced Severe Periodontitis
Localized Periodontitis: Loss of interproximal attachment and bone loss on two permanent teeth, one of the first molar and involving no more than two teeth other than first molars and incisors. Generalized Periodontitis: Loss of adhesion interproximal affecting at least three permanent teeth than the first molars and incisors
