PTI 2018 PERFORMANCE STORIES What this conversation taught us… Interview summary: “It’s an amazing feeling to be a part of a team that is transforming care to a new level. Patients are creating their own ‘patient-to-peer’ support system. ” - Angela Histo, Medical Assistant • PAMF is a participant in the CMS program Million Hearts. Cardiovascular Disease Risk Reduction Model. PAMF developed an organizational program to focus on population health management through shared medical appointments. • Santa Clara Internal Medicine of the 12 pilot sites for SMA (shared medical appointment) visits. • Content of SMA consists of Cardiovascular Disease Education, shared decision making and patient individualized action planning. • SMA Team consists of: Internal Medicine Physician Medical Assistants and a Patient Service Representative. Medical 2 presentation and Assistants take vitals, drive technology scribe sessions live during the group visit. Patient Service Representative greets patients in the lobby, checks in patients and escorts them to the conference room. • Outreach and patient education is key to patient engagement for the sessions. About the practice: “CVD is a topic close to my heart. It changes medicine from a provider giving direction, to the transfer of ideas from one patient to another on lifestyle changes and putting their care in their own hands. ” - Vidhi Shah, M. D.
