Introducing the New PASRR Comprehensive Service Plan (PCSP) Form • Currently providers use separate forms to indicate PASRR services the individual will receive. These forms are the Interdisciplinary Team (IDT) and PASRR Specialized Services (PSS) forms. • A new form called the PASRR Comprehensive Service Plan (PCSP) will replace both the existing IDT and PSS forms. 2

The Benefits of the Form Merger • Clarifies purpose of interdisciplinary team meeting and service planning team meeting. • Provides “one team” approach with nursing facilities and Local Authorities (LAs) to help individuals succeed. 4

The Benefits of the Form Merger Continued • Provides providers and HHSC a comprehensive picture of the progression of PASRR services over a 12 -month period. • Allows HHSC to improve quality monitoring of the initiation of services. 5

Review of Major Changes for LIDDAs 7

Noteworthy Changes • LIDDAs first meeting (initial SPT) will be held on the same date and time as the NFs initial meeting (IDT). • Streamlined one team/one meeting approach will improve continuity of care for individuals served. • LIDDAs will be able to view a 12 -month snapshot of services. 8

Noteworthy Changes • LIDDAs will now be able to view and compare the current meeting outcomes with the previous meeting outcomes on the same document locator number (DLN). • LIDDA staff completing the PE will be able to continue to bill for their attendance at the IDT and the Service Coordinator (later Habilitation Coordinator) will also be able to bill for their attendance at the same meeting. • HHSC PASRR staff will be able to modify both the NFs entries and the LAs entries to ensure the submitted information is accurate. 9

Training Underway • HHSC PASRR staff are scheduled to conduct in-person training beginning 1/11/18 in Austin and continuing through 1/22/19. • A final webinar will be scheduled for 1/23/19. This webinar will be recorded and posted for LAs and NFs to review at any time. 10
