Patient Safety and Quality Care Movement • To Err is Human • Signed into law on July 29, 2005 • First federal legislative attempt to address patient safety • Establish a national medical error reporting system • Avoid error • Goal: ZERO preventable deaths by 2020 (Levy, Mareiniss, Iacovelli, & Howard, 2010)
