What is PCDH? • A method of exchanging patient data among Health Information Exchange’s (HIE) • It is based on triggering episode alerts to notify providers a care event has occurred outside of the patient’s “home” HIE. • It confirms the specific location of the clinical data, enabling providers to initiate an exchange to access real-time information across state and regional lines. © 2020 Quality Health Network Ⓡ (QHN) – All rights reserved, QHN proprietary and confidential not for further redistribution.

What do I need to do to get alerted when my patient goes outside of their medical home? • Access to QHN • Ability to query the longitudinal health record (QHN Results) • Receive ADT (Admit, Discharge, Transfer) event notifications • Set up Patient Subscription © 2020 Quality Health Network Ⓡ (QHN) – All rights reserved, QHN proprietary and confidential not for further redistribution.
Why Subscription? • Patient Subscription is enabled in many of our organizations connected to QHN to ensure they are notified when a patient presents to the Emergency Room (ER) or an Inpatient setting (IP). • Subscription allows organizations to enable a patient panel (subscription) for QHN to alert the organization on an event. • Subscription is used for those cases when the treating facility has not properly identified the Primary Care Provider (PCP). • Patient panel list is sent securely to QHN monthly © 2020 Quality Health Network Ⓡ (QHN) – All rights reserved, QHN proprietary and confidential not for further redistribution.
How do I know my patient went to the hospital? • When alerts/subscription are enabled; a real-time message is sent to an individual or group that has been identified by the organization. • QHN Direct Secure E-mail • Interfaced into Electronic Medical Record (EMR) • Other • When querying a patient’s record, encounters are displayed in the Encounter section of QHN Results that will identify where the patient has been to receive care. © 2020 Quality Health Network Ⓡ (QHN) – All rights reserved, QHN proprietary and confidential not for further redistribution.
