IDT MEMBERS-Visit Frequencies HSP 5 -4 A 418. 56(c)(2) L 547 The hospice develops an individualized written plan of care for each patient in collaboration with the attending physician (if any), the patient or representative, and the primary caregiver in accordance with the patient's needs if any of them so desire. The plan of care must reflect patient/family goals and interventions based on the problems identified in the initial, comprehensive, and updated comprehensive assessments. The plan of care must include all services necessary for the palliation and management of the terminal illness and related conditions. Deficiency: Upon patient record review, patient record did not have evidence the plan of care includes all services necessary for the palliation and management of the terminal illness and related conditions, including the following: • A detailed statement of the scope and frequency of services necessary to meet the specific patient and family needs

Netsmart: PHCC Visit Frequency Background: There must be visit frequencies in for each discipline. Starting on May 29 th RN, SW, CH should have VF entered. LPNs will be coming shortly. Below are the instructions for entering in the minimum amounts of visits. In addition Pathways Hospice Inpatient admission orders must be opened with I 121 intervention opened for GIP level of care.

Visit Frequency 1. Click the [Visit Frequency] tab. 2. Verify you are working in the correct Certification Period tab (across the top) 3. Click [Add a row] (located right side of the screen) for the visit frequency under Regular Visits 4. Click the drop down - and select the appropriate discipline from the list. SN = Nurse (1 -2 x day x 7 days) a. b. MSS = Social Worker c. SC = Chaplain (1 x week x 1 week)

5. Click the ellipsis to define parameters. a. Under [Visits] enter the following: 1. Low will be the least amount of visits being made during the day– 1 2. High will be the greatest amountof visits being made during the day– 2. b. Under [hours]- leave blank c. Under [Duration] enter the following: 1. Period: click day/week from the drop-down. 2. For: enter duration (see above for Disc specific) d. Click [OK].

PRN Visit Frequency 1. Click [Add a row] (located right side of the screen) for the visit frequency under PRN visits 2. Click the drop down - and select the appropriate discipline from the list. 3. i) MSS = Social Worker ii) SC = Chaplain Click the ellipsis to define parameters i) A new window opens. 4. Under [Value] enter 6. 5. Under [Reason] enter psychosocial support or spiritual support (a reason must be entered for PRN visits). 6. Click [OK]. i) Under the [Start] enter the start date for the PRN Visit Frequency. Leave the end date blank ii) Click [SAVE] once you have finished.
