Centralized Access Point (CAP) Access for families that fits their needs Help Me Grow Family Advocates conduct in-person developmental screenings and provide connection to comprehensive services for families and children birth through age 5. Family Advocates work with families who need more hands on support, conducting home visits and creating Family Action plans individualized to the families need.

The Help Me Grow Team Will: • Respond to questions regarding children’s development • Provide a free developmental screening and appropriate resources • Help connect families to early intervention, special education and behavioral health services. • Provide extensive follow-up to families to ensure connection to services.

Family and Community Outreach Importance of Community Partners • Inform families about developmental milestones, developmental screenings, and their importance. • Embed developmental screenings into support systems. • Conduct outreach to build awareness of services and supports for early identification and intervention.
