Recommended Screening Strategies to Prevent Preterm Birth • No current data support use of home uterine-activity monitoring or bacterial vaginosis screening. • Screening for risk of preterm labor, other than historical risk factors, is not beneficial in the general obstetrical population. • Sonography to determine cervical length and/or fetal fibronectin level measurement may be useful in determining women at risk for preterm labor. However, their value may rest primarily with their negative-predictive value given the lack of proven treatment options.

Nausea and Vomiting in Pregnancy
The Clinical Problem • 50 % have nausea and vomiting in early pregnancy • an additional 25% have nausea alone • “morning sickness” • lead to dehydration and weight loss • hyperemesis gravidarum – Incidence 0. 3 to 1. 0 – characterized by persistent vomiting, weight loss of more than 5%, ketonuria, electrolyte abnormalities (hypokalemia), and dehydration (high urine specific gravity).

Evaluation • gastrointestinal conditions (e. g. , appendicitis, hepatitis, pancreatitis, or biliary tract disease), pyelonephritis, and metabolic disorders such as diabetic ketoacidosis, porphyria, or Addison’s disease. • An onset of nausea and vomiting more than 8 weeks after the last menstrual period is rare in pregnancy. • The presence of fever, abdominal pain, or headache is atypical in women with hyperemesis and suggests another cause.
Nausea and Vomiting in Pregnancy Is Not Always Nausea and Vomiting of Pregnancy • While causes of nausea and vomiting unrelated to pregnancy can occur at any time, preeclampsia, HELLP syndrome and acute fatty liver of pregnancy typically occur in the third trimester.
Signs • • changes in blood pressure and heart rate. fever meningismus tenderness, guarding, rigidity, peritoneal signs; right upper quadrant • jaundice.
Lab • urinary • ketones, blood urea nitrogen, creatinine, alanine aminotransferase, aspartate aminotransferase, • electrolytes, amylase • Ultrasonographic testing should be performed to detect multiple gestation or hydatidiform mole.
Management
Pharmacologic Therapies • 10% of women with nausea and vomiting in pregnancy require medication
Management of Refractory Cases • Intravenous hydration and nutritional supplementation. • Enteral tube feeding • Total parenteral nutrition – substantial risk of line sepsis (25%); – steatohepatitis • reserved for clinically significant weight loss (>5% of body weight) and no response to antiemetic regimens or cannot be managed with enteral feedings.
Areas of Uncertainty • The cause or causes remain unclear. • The mechanism of action of vitamin B 6 is unknown. • data are lacking to identify factors predicting the response to therapies. • Vitamin B 6 levels do not predict the response to therapy with vitamin B 6.
Conclusions and Recommendations • Consider other causes of nausea and vomiting in early pregnancy • Dietary advice • vitamin B 6 [pyridoxine], 10 to 25 mg every 8 hours, and doxylamine, 25 mg at bedtime and 12. 5 mg each in the morning and afternoon • Methylprednisolone should be reserved for refractory cases after 10 weeks of gestation. • Alternative remedies such as ginger and acupuncture may be tried at any time.
• intravenous fluid replacement with multivitamins, especially thiamine • If, after 12 hours of intravenous therapy, the vomiting continues, hospitalization may be required. • Enteral 48 or parenteral nutrition should be reserved for patients in whom weight loss continues despite pharmacologic therapies.
THE MANAGEMENT OF NAUSEA AND VOMITING OF PREGNANCY • 1. Dietary and lifestyle changes should be liberally encouraged, and women should be counselled to eat whatever appeals to them. (III-C) • 2. Alternative therapies, such as ginger supplementation, acupuncture, and acupressure, may be beneficial. (I-A) J Obstet Gynaecol Can 2002; 24(10): 817 -23.
• 3. A doxylamine/pyridoxine combination should be the standard of care, since it has the greatest evidence to support its efficacy and safety. (I-A) • 4. H 1 receptor antagonists should be considered in the management of acute or breakthrough episodes of NVP. (I-A) J Obstet Gynaecol Can 2002; 24(10): 817 -23.
• 5. Pyridoxine monotherapy supplementation may be considered as an adjuvant measure. (I -A) • 6. Phenothiazines are safe and effective for severe NVP. (I-A) • 7. • 8. Corticosteroids should be avoided during the first trimester because of possible increased risk of oral clefting and should be restricted to refractory cases. (I-B) • 9.
