DEFINITION Osteoarthritis is a generative disease of cartilage with charecteristics clinical and radiological features affecting small or large joints and the spine. Classification – Primary Osteoathritis. - Secondary Osteoathritis. Primary osteoathritis-mainly occurs in elderly patients. Secondary Osteoathritis-Any age group can be affected.
PATHOGENESIS (a)Mechanical factors-Abnormal bone -Osteopaeni - Trauma - Osteonecrosis. - Pagets disease - Abnormal cartilage-Trauma - Pseudo gout. - Septic arthritis. - TB. - Abnormal load - DDH. - Malignment. -Trauma. -Neuropathy. (a)Chemical factors.
DIAGNOSIS. -History –pain - Stiffness - Crepitus - Deformity-valgus - Varus - Swelling -Examination- Swelling - Derfomity - Muscle waisting - Joint tenderness - Range of joint motion - Crepitus.
q. Differential diagnosis – OA - RA - Gout. q. Investigations - X-Rays – joint space narrowing - Subchondral cyst. - No osteopaenia.
Management Prevention - General fitness. - Correction of angular derfomities - Proper treatment of fractures. - Genetic engineering. Conservative - Life style modification. - Walking stick - Weight reduction. - Physiotherapy - NSAIDS.
Surgery - intra-articular injections. - Hyaluronic acid (synvisc - Joint debridement. - Osteotomy. - Arthropasty - Arthrodesis
