Occupational Therapy Specialty Clinic Referral Why am I being referred 352 -273 -6044 to Occupational - Therapy? equalaccessclinic. ot@gmail. com Some people are born with conditions, suffer injuries, or have chronic conditions that prevent them from taking care of themselves in their usual way. Basic tasks like going to the bathroom, getting dressed or taking a shower may be more difficult due to changes in how you think or move caused by your condition. You might also struggle with more complicated tasks like preparing meals, managing finances, engaging in your hobbies, managing your household, driving or other tasks. Occupational Therapists are trained to help people engage in the “job of living”. Through specific exercises or adapting the task, Occupational Therapists can help you engage more fully in your life. Patient Name: Appointment Date/Time: *The Occupational Therapy Clinic is held on the 1 st and 3 rd Thursday of every month from 6 -9 pm. Please arrive at least 15 minutes prior to your scheduled appointment. Appointments are not required - walk-ins are always welcome. Please be aware that patients without an appointment are seen on a first come-first served basis and the wait time to see a clinician cannot be guaranteed. Please arrive at 6 pm to be fit into the schedule for the evening. Helpful Tips: q Please bring copies of any medical records you have related to your condition/concerns, including surgeon’s reports, x-rays, labs, evaluations/discharges completed by PT/OT/Speech or anything else you may have. q Please bring a list of your primary concerns, including the activities that most challenging for you. q Wear loose, comfortable clothing. It is recommended that you wear an undershirt/tank top to maintain your privacy, in the event your therapist needs to access your shoulders or back as part of your evaluation/treatment. q If you use any assistive devices to help you get around, such as a walker/cane, please bring it with you – even if you only use it “sometimes”! q Caregivers are encouraged to attend and participate in sessions for family training.
