Assessing the Risk • Select four risks caused by hazards at your workplace. • eg. Hearing loss from loud equipment • Complete the first column in Table 1 • Complete the rest of the table using the Risk Rating information

Reporting Hazards, Accidents and Incidents • Report forms are available from the office. • The person who is involved or is a witness fills out the form. • The completed form is either given to lecturer(your supervisor or the safety department). • Complete the Hazard report form.
